The CHCollective Purchasing Summit & Reverse Expo

By checking this box, my community health center commits to participating in the Reverse Expo at the CHCollective Purchasing Summit.

This commitment includes hosting a booth for a portion of the Reverse Expo and having at least one representative present at all times during the designated Expo hours.

In exchange for our participation, my health center will receive free registration for up to [X] attendees to the entire CHCollective Purchasing Summit.

If circumstances arise that prevent our participation, I agree to notify CHCollective in writing at least [X] days prior to the event. Late cancellations or no-shows will still be subject to the full registration fee.
